9/26/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a flat state as evidenced by greeting QMHP-C with a lack of emotions. QMHP-C engaged the client in discussion surrounding his flat state. The client explained that he was fatigue as a result of getting limited sleep overnight. During morning observations, the client exhibited difficulty with controlling impulsive behavior by not staying in his seat during instructional periods. During midday observations, the client continued to express difficulty with staying in his assigned lunch area. During afternoon observations, the client exhibited difficulty with all of his target goals to include disruptive behaviors and avoiding to complete assignments. The client expressed great difficulty staying in his assigned area, communicating in an inappropriate manner with authority, and refusing to follow directions. QMHP-C greeted the client upon arrival to the school setting. `During morning observations, QMHP-C redirected the client to stop running throughout the classroom. QMHP-C reiterated the importance of controlling impulsive behavior and staying in his seat. During midday observations, QMHP-C redirected the client and reiterated the importance of staying in his assigned lunch area. During afternoon observations, QMHP-C redirected the client and reiterated the importance of achieving all of his target goals. The client responded well towards TDT services as evidenced by participation and engagement. During morning observations, the client was unreceptive to redirection by running through the classroom. The client was then able to calm down and agreed to go back to his resource class. During midday observations, the client was receptive to redirection and agreed to stay in his designated lunch area. During the afternoon observations, the client was unreceptive to redirection by yelling and using inappropriate language. The client was then able to calm down and agreed to work towards achieving all of his target goals. QMHP-C will continue to assist the client with eliminating disruptive behaviors within the classroom to include throwing objects, talking out of turn and interrupting peers. QMHP-C will continue to encourage the client to utilize appropriate safety precautions and engage in healthy activities to help and maintain stability through COVID 19. QMHP-C will continue to provide support to the client within the school setting in an effort to maintain in school learning.
